Treatment planning:

Once your treatment strategy has been agreed with the Dentist, we'll supply printed details of your strategy. Whether this strategy changes on account of clinical or radiographic findings, you will be informed by us and discuss this with you. Treatment plans are valid for net 90 days from your date. Please do not hesitate to ask us, before the appointments for actual treatments are made if there are any points in your treatment strategy which you wish to query.

Approval forms:

Particular treatments necessitate completion of a written consent form. This is before any of these treatments are carried out, to be able to explain the treatment, aftercare and any danger to you personally completely.

Fees:

A credit account system doesn't operate and we require fees to be settled at the appointment.
At least 50% of the total fee is due at the appointment where impressions are shot where a lab fee is incurred by treatment. Fees for certain treatments like Invisalign and Dental Implants are taken in staged payments at each visit.

Where treatment is paid for on finance through a Finance company, please take note that the finance agreement must be accepted and signed just before the start of that treatment. Please note that finance is exclusively available on treatments over?400, not including Emergency Dental Treatment, and a minimal deposit may also be demanded.

If after signing the deal forms you have funded some of the prices of your treatment through the finance company, and wish to cancel your treatment, please take note that the cancellation fee of up to 15% of the entire finance amount will be billed.

Pearl Dental Clinic doesn't accept any payments by cheque.

Fees for treatment where intravenous sedation is contained, must be paid prior to the appointment to avoid fiscal trades, or signatures being demanded, whilst a customer remains under the influence of the sedative.

Deposits are deductible off the price of treatment.

Dream Smile Dental Clinic reserves the right to make a charge for any debt passed to a debt collection agency.

All the fees applied for any form of Dental treatment at the practice are based on; the materials used, lab costs incurred (if applicable), and the clinical time spent. The fees are however, illustrated as treatments prescribed on the treatment strategy for your information.

Late cancellation or appointments that are missed:
Dream Smile Dental Clinic reserves the right to charge up to?100 per hour, in the function of a missed appointment or an appointment. For appointments more than 1 hour, we require at least 72 hours.

We understand that some patients travel long distances to get to the practice, and in some events being late for appointments can be inescapable. If you're more than 10 minutes, please take note that you might be requested to reschedule your appointment.

Guarantee:

Ceramic Veneers, at Dream Smile Dental Clinic our Lumineers, Implant crowns and Ceramic crowns are guaranteed for a period of 5 years following placement, provided that the following conditions are satisfied:

The patient doesn't owe the practice any money for the treatment received and has fully paid for the treatment.
Implant the Veneers, Lumineers, Crowns & crowns haven't been damaged as a consequence of a collision, trauma or excessive grinding.
All post treatment maintenance recommendations have been followed by the patient.
The patient has attended and regular assessment every 6 months.
Some treatments may have a guarantee of less than 1 year, and in this situation you will be enlightened by your Dentist either verbally or in writing, or both.

Private Details:

It's extremely important that you simply give a full medical history and details of any drugs you take. Should these change in any way, it is essential that you tell your Dentist. It really is the patient's obligation to notify the practice of any changes in personal details or their medical history.

Use of Images and Xrays:

Dream Smile Dental Clinic may use pictures and x-rays of your smile and teeth just, for promotion and educational purposes on the Dream Smile Dental Clinic site, and on promotional and educational literature. Your name WOn't ever be printed, and identity WOn't be disclosed. But if you DO NOT wish for us to use your pictures and x-rays in this way, please let us know.

We like to remind our patients in their appointments, when they've been due for appointments, and other various significant reminders. On this note, you may be occasionally contacted by the practice via mobile, text, email or by letter in the post. Please let us know, if you DO NOT wish to be contacted by the practice by any or all these means.

Complaint's policy:

At Dream Smile Dental Clinic we constantly take complaints about any aspects of our services very seriously, to be able to ensure that each and every patient has just the best experience whatsoever times.



Every complaint will receive immediate written/e-mail acknowledgment, and the Management will strive to resolve the grievance within a fast, fair time period (normally about 2 weeks). For our full complaints process and policy, please request an associate of our reception team. If a grievance is about any aspect of clinical care or affiliated charges it then referred to the treating Dentist and will usually be acknowledged by the Management team.

If you're unsatisfied with our internal management of your complaint, you're free to forward your complaint to the Dental Complaints Service (www.dentalcomplaints.org.uk). If still unsatisfied, you may wish to forward your complaint to the General Dental Council.

No forbearance/Mistreatment policy

At Dream Smile Dental Clinic we operate a zero tolerance policy to abuse to our Dentists and staff, loud/disorderly/drunken behaviour, persistent overlooking and late cancellation of appointments (after multiple warnings). In these scenarios, the right to deny treatment and admission is reserved by Dream Smile Dental Clinic.

Data Protection Act:

We store all patient personal details on a secure computer system in accordance with the Data Protection Act. All clinical notes, digital radio-graphs photos etc that are digital, remain the property of Dream Smile Dental Clinic. Copies of radiographs, notes and photographs can be made available on request, and the right to charge an administration fee is reserved by Dream Smile Dental Clinic.
